The Postal Service requires this form from everyone who receives mail through a mailbox provider. Do it online in about ten minutes, or fill it in by hand — either way you finish at our counter, because USPS requires us to witness your signature in person.

Download PS Form 1583, complete it, and bring it in with your two forms of ID. Leave the signature blank — you sign it in front of us.
A government-issued photo ID, plus a second document showing your home address — a lease, insurance policy, vehicle registration or voter card. A driver's license counts as only one of the two.
